Myrtle Ridge Primary Care PA, originally known as Myrtle Ridge Family Medicine, was founded on faith, perseverance, and compassion. The story of its growth is one of dedication, opportunity, and divine purpose.


Our founder, Yudaisy Mogena Padron, DMSc, PA, CBHCMS, began her journey at Myrtle Ridge as a Medical Assistant. Her genuine love for patient care, strong work ethic, and leadership quickly impressed the clinic’s former owner, Dr. Berk, who entrusted her with greater responsibilities and eventually promoted her to Clinic Manager.


As Dr. Berk approached retirement, he sought someone who would care for his practice and patients with the same integrity and heart that he had poured into Myrtle Ridge. After receiving several offers, he ultimately decided to entrust his life’s work to two young immigrant doctors from Cuba—Dr. Yudaisy Mogena Padron and her husband, Dr. George Marcel Dorta Chapin, DMSc, PA.


He saw in them the same passion, empathy, and commitment that had defined his practice for decades. For Dr. Berk, Myrtle Ridge was more than a medical office—it was his legacy, his family, and his mission. By passing it on to Yudaisy and George, he ensured that legacy would live on in good hands.


For Yudaisy and George, Myrtle Ridge was a promise from God—a gift they have cared for and cultivated with love, integrity, and faith. They embraced the slogan “Blessed to Care,” recognizing that their work is not only a profession but a calling to serve others.


Today, after five years of continuous growth, Myrtle Ridge Primary Care PA has evolved into a multidisciplinary healthcare center that provides primary care, psychiatry, psychotherapy, targeted case management, and psychological evaluations. Our team includes licensed clinical social workers, family and marriage therapists, master-level clinicians, and dedicated case managers, all united by one mission—to serve the community with excellence and compassion. 


We have also established a partnership with the Department of Children and Families (DCF) to extend support to individuals and families in greatest need, ensuring that no one is left without access to help, hope, and healing.

Willian Torrez Perez

Medical Director & Family Physician MD

Dr. William E. Torres Pérez is a bilingual, board-certified medical doctor with over 11 years of experience in primary and preventive care. He earned his M.D. from the Autonomous University of Guadalajara (2007) and a B.S. in Biology from the University of Puerto Rico.


He specializes in comprehensive, patient-centered care focused on prevention, early diagnosis, and the management of acute and chronic conditions, including diabetes, hypertension, thyroid disorders, obesity, asthma, COPD, depression, and anxiety.


Dr. Torres holds multiple certifications, including Buprenorphine Treatment (DATA 2000), Wound Care, Florida Low-THC Cannabis, and Medical Director credentials, as well as ACLS, BLS, PALS, and ATLS. He is also trained in Botox, dermal fillers, laser therapy, anti-aging medicine, and weight management, offering an integrative approach to wellness.


Known for his compassionate and professional care, Dr. Torres is dedicated to educating and empowering patients while building lasting relationships based on trust. Fluent in English and Spanish, he proudly serves Florida’s diverse community with a commitment to clinical excellence and patient well-being.

Yudaisy Mogena

Physician Assistant PA

Yudaisy Mogena Padron, PA, is a dedicated and compassionate Physician Assistant providing personalized primary care for adults and adolescents. Originally trained as a physician in Cuba, she combines extensive medical expertise with a patient-centered approach focused on prevention, early detection, and effective management of various health conditions.


As co-owner of Myrtle Ridge Primary Care in Lutz, FL, she offers services including chronic disease management, women’s health, mental health care, wellness exams, hormone therapy, and telehealth. Fluent in English and Spanish, Yudaisy is passionate about empowering patients to take charge of their health.

George Dorta Chapin

Physician Assistant PA

George M. Dorta, PA, is an experienced and compassionate Physician Assistant providing comprehensive primary care for adults and adolescents. Originally trained as a physician in Cuba, he brings extensive medical expertise and a patient-focused approach to his practice.


As co-owner of Myrtle Ridge Primary Care in Lutz, FL, George specializes in chronic disease management, preventive care, behavioral health, hormone therapy, and telehealth services. Fluent in English and Spanish, he is dedicated to building trust, promoting wellness, and supporting long-term health for every patient.

Taniabex Lopez Garcia

Therapist

Taniabex is a bilingual mental health professional dedicated to providing compassionate and culturally responsive care. She holds a Master’s Degree in Clinical Social Work and is a Registered Clinical Social Worker Intern (RCSW) with more of 7 years of experience serving children, adolescents, adults, and older adults.


Currently pursuing a PhD in Social Work, she specializes in individual and group therapy, supporting clients with evidence-based strategies that strengthen resilience and improve quality of life.

Yenima Hernández Rodríguez

Therapist

Yenima earned her degree in Psychology in Cuba and brings more than 17 years of clinical experience. She holds a Master’s degree in Community Mental Health and Sexuality, is certified as a Clinical Sexologist and Sexual Therapist, and completed her Doctorate in Psychology in Ecuador.


In the United States, she practices as a Master’s Level Practitioner, specializing in Third-Generation Contextual Therapies such as FAP, ACT, DBT, BA, IBCT, and Mindfulness-based approaches, supporting individuals and couples in building resilience, strengthening relationships, and enhancing overall well-being.

Rosario María Fraga Gómez

Therapist

Rosario is a physician and psychiatrist with a Master’s in Clinical Psychology from the University of Havana. She brings over 30 years of medical experience and 23 years in psychotherapy.


In the United States, she practices as a Master’s Level Practitioner, specializing in individual, family, couples, and group therapy.


Her approach integrates Narrative Therapy, CBT, DBT, and Reflective Dialogic practices to foster resilience, emotional well-being, and personal growth.

Denisleydis Denis

Clinical Director of Mental Health LCSW-SCBHCMS

Denisleydis is a Licensed Clinical Social Worker with over 10 years of experience supporting high-risk populations. A Barry University graduate, she has worked across psychiatric units, outpatient therapy, PHP, IOP, case management, and community outreach.


Specializing in suicide prevention, she is passionate about helping adolescents struggling with self-harm. She also founded 988 in Spanish to expand mental health resources for the Hispanic community.


Guided by compassion, cultural understanding, and evidence-based care, she is dedicated to walking alongside individuals on their path to healing.
